We conduct an annual compensation review for our employees. Three weeks prior to the scheduled compensation review meeting, we send out our compensation review form that the employee fills out. This submission is reviewed by our leadership team (the compensation committee) and factors into our comp decisions along with other data like supervisor's notes on the employee's performance as well as our quarterly one-on-ones.
The form is very simple and consists of four primary long-form text field sections. We've kept to this format for several years now and have found that it satisfies our needs.
We've put this into a Word document (.docx). We recommend putting it into a form/survey software like Google Forms, Typeform, or Survey Monkey.
